A sliding fee scale (or sliding pay scale) provides discounted costs for medical and dental visits based on a person’s household income.

The sliding fee scale is available to patients who don’t have insurance (uninsured), and patients who are underinsured (when the cost according to the sliding fee scale would be less than an insurance copay or deductible).

How the Sliding Fee Scale Works

The sliding fee scale helps lower the cost of care for patients who do not have insurance or whose insurance does not cover all services. Discounts are based on household income and family size and are available for both medical and dental care.

How to Qualify

You may qualify for the sliding fee scale if:

  • You do not have insurance or are underinsured
  • Your household income meets federal guidelines
  • You provide proof of income for your household

Patients who quality for the sliding fee scale discount could pay as little as $20 for a medical visit or $40 for a dental visit.

How to Apply

To apply, you will need to complete a one‑page application and provide proof of income. Applications are available online or at the front desk when you arrive for your appointment.

Proof of income must be provided within 30 days of your visit for the discount to apply. Once approved, your discount is valid for 12 months and must be renewed each year or if your income changes.
