We believe that primary and preventative healthcare should be provided at a cost that anyone can afford. That’s why we accept most insurances, including private insurance, Medicare, and Medicaid. See the full list of insurances we accept below.

As a Federally Qualified Health Center (FQHC), we don’t turn anyone away because of their inability to pay. We offer a sliding fee scale based on household income, you could pay as low as $20 for your visit.

Payment & Insurance Questions

Yes, we accept traditional Medicaid as well as the Medicaid Managed Care Plans (AmeriHealth, Anthem, Buckeye, Caresource, Humana, Molina, and United Healthcare).

Yes, we accept traditional Medicare as well as most of the Medicare Advantage Plans (Part C).  Call our billing office to see if we are contracted with your plan at 937-586-9733, option 7.

Yes, we accept patients who don’t have insurance. We offer a sliding fee scale, which is a discount program for our services based on your income level. Click here to learn about our sliding fee scale.

A sliding fee scale, or sliding pay scale, is a discount on our services based on your income. This means that you could pay as little as $20 for a medical visit, and $40 for a dental visit.

You can still be seen, even if you can’t pay at the time of your visit. If you have questions about paying your bill online please contact our Billing Department at 937-586-9733, option 1, then option 2, Monday – Friday, 9:00 am – 4:00 pm.

You can apply for our sliding fee discount program by filling out the sliding fee scale application. You can print it out and bring it with you, or fill it out at your next doctor’s appointment.

Copays and payment are due at your appointment checkout. If you can’t pay right then, you can pay your bill online.
